COVID-19 self-assessment

Symptoms of COVID-19, the disease caused by the 2019 novel coronavirus, range from mild—like the flu and other common respiratory infections—to severe. If even one of the symptoms related to COVID-19 is present YOU ARE NOT PERMITTED to attend church. No symptoms listed below involve an existing condition.

Are you currently experiencing any of these symptoms?

• Severe difficulty breathing (struggling for each breath, can only speak in single words)

• Severe chest pain

• Feeling confused or unsure of where you are

• Losing consciousness

• FeverTemperature of 37.8 degrees Celsius/100 degrees Fahrenheit or higher

• Chills

• Cough that's new or worsening

• Barking cough, making a whistling noise when breathing

• Shortness of breathOut of breath, unable to breathe deeply

• Sore throat

• Difficulty swallowing, Painful swallowing

• Runny nose

• Stuffy or congested nose

• Decrease or loss of taste or smell

• Pink eye

• Headache that’s unusual or long lasting

• Digestive issues like nausea/vomiting, diarrhea, stomach pain

• Muscle aches that are unusual or long lasting

• Extreme tiredness that is unusual

• Falling down often

• Sluggishness or lack of appetite

• Tested positive for covid-19

In the last 14 days, have you been in close physical contact with someone who currently has COVID-19?

• This includes getting a COVID Alert exposure notification.

In the last 14 days, have you been in close physical contact with someone who either:

• is currently sick with a new cough, fever, difficulty breathing, or other symptoms associated with COVID-19?

• returned from outside of Canada in the last 2 weeks?

Have you travelled outside of Canada in the last 14 days?

This does not include essential workers who cross the Canada-US border regularly.
